Getting Back to Curling – Required Documents

Mandatory Vaccination Requirement

As we heard on the news last week, the Province of Ontario is mandating COVID-19 vaccinations to enter select settings. This means as of September 22nd only individuals who show proof of vaccination – vaccine receipt and government ID (14 days after second shot) will be able to enter the Huntley Curling Club. We need to collect and verify this information before you start curling.

Documentation

There are two documents – COVID Declaration and Participation Waiver – all members must read and sign before entering the curling club.

Please print copies of each document below, review it, and sign it. These documents must be dropped off at the curling club BEFORE curling begins. Only hard copies will be accepted, no soft copies. When you drop off your documents, be prepared to also show your COVID proof of vaccination.

Process

Print and sign the appropriate above documents. Visit the club at one of the following times with your signed documents AND Proof of Vaccination – you don’t need to schedule an appointment, just drop by:

  • October 7 – 1:00 pm to 8:00 pm
  • October 8 – 1:00 pm to 8:00 pm
  • October 9 – 12:00 pm to 5:00 pm
  • October 11 – 1:00 pm to 8:00 pm

If you really can’t make any of the above times, bring your paperwork to your first game. We recommend you come early to allow for enough time to process your paperwork before your game starts!

If you have a medical exemption for the COVID-19 vaccine please contact the membership director, David Bird, at membership@huntleycurling.ca

Volunteers Needed

Proof of Vaccination & Document Collection – we will need volunteers to be at the curling club at these scheduled times, to verify member vaccination paperwork and collect each member’s signed Waiver, COVID Declaration, and (only for those under 26) Concussion Awareness acceptance.  Sign up to volunteer.
